بررسیهای ناهنجاریهای دستگاه ادراری تناسلی جنین

<strong><span style="font-family:b zar;">چکیده</span></strong><br>
<strong><span style="font-family:b zar;"><span style="font-size:8.0pt;">ناهنجاری های مادرزادی دستگاه ادراری در 1/0٪ تا ۱ % تمامی نوزادان رخ میدهد و یک علت اصلی موربیدیتی و مورتالیتی پری ناتال بشمار می رود. امروزه اکثر آنها با اولتراسونوگرافی تشخیص داده می شود در این مطالعه ناهنجاری دستگاه ادراری قبل از تولد و بررسی علل آن در بعد از تولد مورد بررسی قرار گرفته است. </span></span></strong><br>
<strong><span style="font-family:b zar;"><span style="font-size:8.0pt;">در این مطالعه ۷۲۰۰ زن حامله در سه ماهه سوم را که نیاز به سونوگرافی پریناتال پیدا کرده بودند قرار گرفت. از این تعداد 66 مورد ( 9/0%) دچار ناهنجاری دستگاه ادراری تناسلی بودند، این ناهنجاری ها در جنس مذکر شایعتر از جنس مؤنث بود ( 5/1 برابر). در این بررسی شایعترین یافته پره ناتال هیدرونفروز ۵۹ مورد ( 3/89%) بود که ۲۱ مورد ( %5/35) آن دو طرفه بود که در بررسی بعد از تولد شایعترین علت هیدرونفروز تنگی محل اتصال حالب به لگنچه یعنی ۴۲ مورد (7/63%) بود و بعد از آن ریفلاکس، ۹ مورد (6/10%) والو دریچه خلفی، ۴ مورد (6%) و تنگی محل اتصال حالب به مثانه، ۴ مورد (6%) بود اولیگوهیدرا آمنیوس در ۵ مورد ( 6/7٪) یافت شد. دیلاتاسیون دستگاه ادراری فوقانی شایعترین ناهنجاری دستگاه ادراری تناسلی بود و تنگی محل اتصال حالب به لگنچه شایعترین علت هیدرونفروز آن بود که تنگی محل اتصال حالب به لگنچه در هر دو جنس از شیوع تقریبا" مساوی برخوردار بود. <strong>ABSTRACT <span dir="RTL"></span></strong><br>
Congenital genitourinary tract abnormalities occur in 0.1% to 1% of all neonates and are a major cause of perinatal mortality and morbidity. Nowadays, prenatal ultrasonography can detect most malformations and anomalies. In this study the genitourinary tract abnormalities of the Fetuses of the pregnant women by prenatal ultrasonography and causes of these abnormalities were evaluated. 7,200 pregnant women in 3rd trimester who need specific ultrasonographic evaluation was selected. The fetuses of these pregnant women evaluated for geritourinary abnormalities and causes of these anomalies. <span dir="RTL"></span><br>
66 (0.9%) of cases had genitourinary anomalies. The male to female ratio was 1.5/1. Hydronephrosis was the most common abnormality detected and accounts for 59 (89.3%) of all prenatally detected lesions. 21 (35.5%) of these had bilateral hydronephrosis. In postnatal evaluation, 42 (63.7%) of prenatally detected hydronephrosi was caused by ureteropelvic Junction obstruction then vesicoureteral reflux 9 (10.6%), posterior urethral valve 4(6%), ureterovesical Junction obstruction 4 (6%).<br>
Oligohydroaminous was detected in 5 of patients (7.6%). Upper urinary tract dilatation was the most common genitourinary tract abnormalities and also ureteropelvic junction obstruction was the most common causes of Hydronephrosis.
