بررسی رضایت بیماران و پزشک از جا اندازی بسته شکستگی بینی
Evaluation of Satisfaction Rate of Patients and Physician from Closed Reduction of Nasal Fracture

چکیده مقدمه: شکستگی استخوان بینی شایعترین شکستگی در صورت و سومین شکستگی اسکلت استخوانی است. تشخیص شکستگی بینی و انتخاب درمان مناسب تا حد زیادی به نظر و مهارت پزشک بستگی دارد. نوع درمان بیماران میتواند بهصورت جااندازی بسته یا باز باشد. در صورت جااندازی نامناسب پس از فروکش ادم، بدشکلی ثانویه باقی خواهد ماند که موجب درخواست اعمال جراحی بعدی میشود. هدف: تعیین میزان رضایت بیمار و پزشک، یک ماه پس از درمان شکستگی بینی به روش بسته. مواد و روشها: این مطالعه در سالهای 85 و 86 به صورت توصیفی رو به جلو در بیمارستان امیرالمؤمنین(ع) شهر رشت در بیماران دچار شکستگی بینی با درمان جااندازی بسته، انجام شد. با توجه به معاینه بالینی و یافتههای رادیوگرافی، شکستگی بیماران بهصورت جابجایی یکطرفه (U)، دوطرفه (B) و خلفی (F) استخوان بینی طبقهبندی شد. اطلاعات دموگرافی، زمان تروما، مکانیسم آسیب و شدت شکستگی بینی ثبت شد. یک ماه پس از درمان، بیماران ویزیت و معاینه شدند و میزان رضایتمندی بیماران به صورت مقیاس لایکرت و پزشک به صورت خوب، نسبتا خوب و بد ثبت شد. نتایج: در این مطالعه از 67 بیمار،1/76% مرد و 9/23% زن بودند. میانگین سنی آنها 45/13 17/30 سال بود. نزاع و ضربههای مشت بیشترین عامل شکستگی بینی بود(3/43% موارد). 1/70 % بیماران شکستگی نوعB ، 4/25% نوع U و 5/4% نوعF داشتند . 9/26% بیماران همراه با شکستگی بینی، آسیب بافتهای نرم و 3% شکستگی سایر استخوانهای صورت داشتند. 7/53% بیماران(36 نفر) از جااندازی بینی خود راضی، 4/25% آنها (17نفر) بسیار راضی و 9/20% (14 نفر) ناراضی بودند. هیچ یک از بیماران از نتیجه درمان بسیار ناراضی نبودند. میزان رضایت پزشک پس از جا اندازی در 7/65% موارد (44 نفر) خوب ، 9/23% (16 نفر) نسبتا خوب و 4/10% (7 نفر) بد بود. نتیجهگیری: هرچند تفاوت آماری معنیدار بین میزان رضایتمندی بیماران با انواع مختلف شکستگی یا آسیب بافت نرم همراه وجود نداشت، ولی بین میزان رضایتمندی پزشک و انواع مختلف شکستگی بینی و شدت آسیب وارده به نسوج نرم همراه، ارتباط آماری بدست آمد(به ترتیب 02/0= Pو 049/0=P). نتایج نشان داد که در موارد شکستگی شدید (نوع F) و همراهی آن با آسیب بافت نرم در انتخاب جااندازی بسته بینی به عنوان روش درمانی باید دقت بیشتری شود.
Abstract Introduction: Nasal bone fracture is the most common facial fracture and the third most common fracture of the human skeleton. The diagnosis and proper treatment of the nasal bone fracture are based on physician’s findings and skill. The way to manage the nasal bone fracture ranges from closed reduction to open reduction. Insufficient reduction caused secondary deformity after the swelling subside, and lead to requests of other surgical procedures. Objective: To evaluate satisfaction rate of patients and physician from outcome of closed reduction 1 month later nasal fracture Materials and Methods: The descriptive study reviewed patients with nasal fracture and treated by closed reduction at Amir-al-momenin hospital (Rasht) in 2007-2008 years. According to physical examination and radiologic findings, nasal fractures were classified. Demographic data, time of injury, mechanisms and severity of fracture were recorded on data sheet. Patients were followed up 1 month later of nasal fracture and their satisfaction upon Likert scale was recorded. The physician described satisfaction as good, fair, poor. Results: In this study from 67 patients 76.1% were male and 23.9% were female. Mean age of patients was 30.17± 13.45 years. The major cause of the nasal fractures was attributed to fighting (43.3%). B-type, U-type and F-type fractures composed 70.1%, 25.4% and 4.5% respectively. 26.9% of patients had soft tissues damages and 3% had fracture in other facial bone. 36 patients (53.7%) were satisfied, 17 patients (25.4%) were very satisfied and 14 patients (20.9%) were unsatisfied. None of them were very unsatisfied. Physician evaluated outcome as good, fair, and poor in 65.7% (44 cases), 23.9% (16 cases) and 10.4% (7 cases) respectively. Conclusion: Although, there isn’t statistically significant difference between different type of fracture or presence of soft tissue damage and patients' satisfaction rate, there is statistically significant difference between them and physician's satisfaction rate (P=0.02 and P=0.049). The results showed that in cases with severe fracture (F-type) and presence of soft tissue damage, closed reduction should be considered carefully as the choice of treatment.
