بررسی رشد در بیماران تالاسمی ماژور شهرستان رشت

چکیده
مقدمه: رشد روندی پیچیده و وابسته به عوامل متعدد دارد و اختلال آن یکی از مسائل مهم در طب است که پیگیری، تشخیص علل و درمان آن ضروری ست. یکی از این بیماریها تالاسمی ماژور است. گرچه این بیماران به دلیل آنمی مزمنِِ، اضافه بارآهن ناشی از تزریق خون، اختلال هورمونی و علل دیگر دچار اختلال رشد می شوند، اما امروزه با درمان دقیق و منظم تا حد زیاد میتوان از آن جلوگیری کرد.
هدف: ارزیابی وضعیت بالینی و پیگیری دقیق درمانی با نشان دادن میزان اختلال رشد در بیماران تالاسمی شهرستان رشت.
مواد و روش ها: اختلال رشد قدی و وزنی بیماران ( صدک قد و وزن زیر 5 درصد) و برخی عوامل از جمله سن، سن شروع درمان، متوسط تعداد مراجعه به درمانگاه جهت دریافت خون و تعداد واحدهای خون دریافتی در سال با مصاحبه، معاینه و بررسی پرونده آنان اندازهگیری شد.
نتایج: از 392 بیمار، 3/42 درصد قد و 37 درصد وزن زیر 5 درصد داشتند. در مجموع 190 بیمار (5/48 درصد) اختلال رشد قد، وزن یا هر دو را داشتند. دو جنس از نظر اختلال رشد تفاوت معنیدار نداشتند اما با افزایش سن این اختلال افزایش یافت که اکثراً مربوط به دهه دوم زندگی بود. بر اساس نتایج آنالیز آماری logistic regression متغیرهای سن، سن شروع درمان به طور مستقیم و تعداد مراجعه سالانه به طور معکوس با اختلال رشد رابطه معنی دار داشتند.
نتیجه گیری: اختلال رشد بیماران ما در مقایسه با آمار اروپا به طور قابل توجهی بیشتر و در حد جهان سوم است. لذا، پیگیری درمانی دقیق تر این بیماران ضروری می نماید.
Abstract
Introduction: Growth disorder is a complex and multi-factorial problem which needs a precise assessment. In Thalassemia major there is much more growth disorder due to chronic anemia, Iron overload, endocrine dysfunctions and etc which can be prevented by accurate and regular treatment.
Objective: In this study we wanted to evaluate height and weight disorders in order to guess clinical feature and condition of treatment in Thalassemia major patients.
Materials and Methods: we measured height and weight of thalassemia major patients (height and weight < 5% were considered as a disorder) and some factors such as age, the age of first treatment, the number of visits (for transfusion therapy) and the number of transfused blood units in per year by interview, examination and survey of their files.
Results: Among 392 patients, 42.3% of them had height under 5% and 37% had weight under 5%. Totally, 190 patients (48.5%) had growth disorder (height, weight or both). Sex had no significant relation with growth disorder. Most of disorders occurred in second decade of life. According to the logistic regression analysis, age, the age of first treatment had positive relations and the number of visits in per year had a negative relation with growth disorder.
Conclusion: Comparing these results with developed countries showed that we are in a backward state and our patients need more precise treatment.
