یافتههای گوشی و شنوایی سنجی در بیماران با نقص ایمنی اکتسابی انسانی (HIV مثبت)

چکیده مقدمه: ویروس نقص ایمنی انسانی (HIV) می تواند سبب بروز علایم و بیماریهایی در حیطه گوش و گلو و بینی شود. با این وجود گزارشات اندکی در این زمینه از کشورهای در حال توسعه وجود دارد. از سویی دیگر فراوانی بیماران HIV مثبت (علامت دار یا بدون علامت) در کشور ما رو به فزونی است. آگاهی از عوارض این بیماری بر سیستم شنوایی می تواند سبب مداخله زودهنگام و بهبود کیفیت زندگی بیمار شود. هدف: تعیین تظاهر گوشی و شنوایی سنجی در بیماران HIVمثبت مواد و روشها: مطالعه به صورت توصیفی- مقطعی بر 30 بیمار شناخته شدهHIV مثبت در مرکز بهداشتی- درمانی شماره 10 رشت انجام شد. این بیماران بین ماههای فروردین تا اسفند 91 برای بررسی بیشتر تظاهر بیماری در دستگاه شنوایی به بیمارستان امیر المومنین (ع) ارجاع شده بودند. از تمام بیماران شرح حال دقیق گرفته شد، سپس، دستیار تخصصی گوش، گلو، بینی و جراحی سر و گردن معاینه بالینی کامل سیستم شنوایی را انجام داد. در همه بیماران آزمونهای شنوایی سنجی شامل تمپانومتری، شنواییسنجی تن خالص، گسیلهای صوتیگذرای گوش(TOAEs) و پاسخ شنوایی ساقه مغز(ABR) صورت گرفت. نتایج: از 30 بیمار HIV مثبت بررسی شده 21 نفر مرد و 9 نفر زن و از این تعداد 2 نفر وارد مرحله ایدز شده بودند. میانگین سنی بیماران 71/9± 27/36 ساله (محدوده سنی 56-24سالگی) بود. 8 بیمار از علایم گوش شکایت داشتند که شامل سرگیجه (7 مورد)، وزوز گوش (4 مورد)، درد گوش و احساس پری گوش (هر کدام 3 مورد) بود. کاهش شنوایی بر اساس اودیومتری در 8 گوش از 60 گوش ثبت شده (6 بیمار) وجود داشت که 4 مورد هدایتی و 4 مورد حسی- عصبی بود. 21 نفر (70%) ،TOAE غیرطبیعی داشتند. همچنین، در 9 بیمار (30%)، ABR غیرطبیعی دیده شد که شایعترین آن افزایش بین قلهای موجI-III (14 گوش) بود. نتیجهگیری: با توجه به فراوانی بالای اختلال TOAEs در این بیماران و افزایش فاصله بین قلهای امواج I-IIIدر ABRبه نظر میرسد که مکان آسیب گوش، سلولهای مویی خارجی حلزون شنوایی و بخش دیستال مسیر شنوایی مرکزی باشد و انجام TOAEs نسبت به PTA آزمون حساستری بوده و به همراه ABRبتواند در ردیابی زودهنگام کاهش شنوایی این بیماران موثر باشد.
Abstract Introduction: Human Immunodeficiency Virus (HIV) can cause some of the symptoms and diseases in ENT field. However, there are a few reports from developing countries and there is not any report on this issue. On the other hand, the prevalence of HIV positive (symptomatic or asymptomatic) patients is increasing across the nation. Awareness of these complications of hearing system can result in early intervention and improve patients' quality of life. Objective: Determining the otologic and audiologic findings in HIV positive patients Materials and Methods: A descriptive, cross-sectional study was performed on the 30 HIV positive patients referred from number-10 medical-therapeutic center of Rasht to Amiralmomenin hospital from April 2011 to March 2012. The senior resident of ENT- HNS took a comprehensive medical history and performed full clinical examination in the auditory system. In all patients, audiometric tests including tympanometry, pure tone audiometry, transient otoacoustic emissions (TOAEs) and auditory brainstem response (ABR) were done. Results: Of the 30 HIV -positive patients, 21 were male and 9 were female. Of these, 2 patients were in the AIDS phase. The mean age of patients was 36.27 ± 9.71 years (range 24-56), and 8 patients complained of otic symptoms: dizziness (7 cases), tinnitus (4 cases), otalgia and fullness (each 3 cases). In audiometry, we found hearing loss in 8 of 60 ears (6 patients). The conductive and sensorineural hearing losses were found in 4 and 4 ears, respectively. As revealed, 21 out of 30 patients (70%) had abnormal TOAE. Also, in 9 out of 30 patients (30%), we observed ABR abnormality, with increased interpeak of wave I-III (14 ears)being the most frequent. Conclusion: Given the high prevalence of TOAEs abnormality in these patients and increased interpeak of waves I-III in ABR, it seems that the locus of ear damage is the outer hair cells of cochlea and distal part of the central auditory pathway and TOAEs is more sensitive than PTA and with ABR test it can help early detection of hearing loss in such patients.
