مصرف لبنیات و خطر سکته مغزی: یک مطالعهی مورد- شاهدی
Dairy Consumption and Risk of Stroke: a Case-control Study

مقدمه: ارتباط دریافت محصولات لبنی با خطر سکته مغزی هنوز به روشنی مشخص نیست. مطالعات قبلی به بررسی ارتباط مصرف لبنیات و سکته مغزی در جوامع غربی و کشورهای توسعهیافته پرداختهاند و هیچ اطلاعاتی در این زمینه در منطقه خاور میانه وجود ندارد. هدف: این مطالعه مورد- شاهدی به منظور ارزیابی ارتباط میان دریافت محصولات لبنی و خطر ابتلا به سکته مغزی در جمعیت بزرگسال ایرانی انجام شد. مواد وروشها: در این مطالعه 195 بیمار دچار سکته مغزی (که توسط یافته های بالینی و اسکن توموگرافی کامپیوتری تشخیص داده شده بودند) در بخش اعصاب بیمارستان الزهرا دانشگاه علوم پزشکی اصفهان مورد مطالعه قرار گرفتند. گروه کنترل که از نظر سن و جنس همسانسازی شده بودند با روش نمونهگیری آسان غیر تصادفی از دیگر بخش های بیمارستان انتخاب شدند. پرسشنامه بسامد خوراک اعتبارسنجی شده برای ارزیابی دریافت های غذایی معمول شرکت کنندگان مورد استفاده قرار گرفت. اطلاعات مربوط به الگوی فعالیت فیزیکی، سیگار کشیدن، وضعیت اقتصادی اجتماعی و متغیرهای مردم شناختی با استفاده از پرسشنامه جمع آوری شد. نتایج: بیماران مبتلا به سکته مغزی مسن تر بوده، وزن و شاخص تودهبدنی کمتری داشتند، احتمال مذکر بودن آنها بیشتر و احتمال چاقی در آنها کمتر بود. بعد از تعدیل سن، جنس و انرژی دریافتی، کسانی که بالاترین دریافت لبنیات کم چرب را داشتند 42 درصد شانس کمتری برای ابتلا به سکته مغزی داشتند (99/0- 34/0CI: 95% 58/0:OR) در حالی که افرادی که بالاترین دریافت لبنیات پرچرب را داشتند 128 درصد شانس بیشتری برای ابتلا به سکته مغزی داشتند (00/4-29/1 CI95% 28/2 : OR). پس از تعدیل بیشتر برای فعالیت فیزیکی و سیگار کشیدن، ارتباط میان دریافت لبنیات کم چرب با سکته مغزی به صورت حاشیه ای معنی دار بود(07/0=P) ؛ ولی ارتباط میان مصرف لبنیات پرچرب و خطر سکته مغزی تنها مختصری ضعیف تر شد (88/3-24/1: 19/2). ارتباط میان مصرف لبنیات پر چرب و سکته مغزی حتی بعد از تعدیل بیشتر برای متغیرهای غذایی همچنان معنیدار باقی ماند (برای سهک بالایی در مقایسه با سهک پائینی )02 /4- 02/1CI: 95% 02/2:OR)(. نتیجهگیری: یافتههای این مطالعه نشان میدهد که رابطه مثبت معنیداری میان مصرف لبنیات پر چرب و خطر سکته مغزی وجود دارد. مطالعات آیندهنگر بیشتری برای تأیید این یافتهها مورد نیاز است.
Introduction: It remains controversial if dairy product intake is associated with risk of stroke. Previous studies on this issue came from Western populations and developed countries, and no information is available from the understudied region in the Middle East. Objective: This case-control study was conducted to assess the relationship between dairy consumption and risk of stroke in an Iranian adult population. Materials and Methods: In this study, 195 stroke patients (recognized based on clinical findings and computed tomography scan) hospitalized in neurology ward of Alzahra University Hospital were enrolled. Sex- and age-matched controls (n=195) were selected with convenience non-random sampling procedure from other wards of the hospital. A validated food frequency questionnaire was used to assess participants' usual dietary intakes. Data on usual physical activity patterns, smoking and socioeconomic status and demographic features were collected using of questionnaires. Results: Patients with stroke were older, had lower weight and BMI and were more likely to be male and physically active and less likely to be obese. After adjustment for age, sex and total energy intake, individual with the highest consumption of low-fat dairy had 42% lower chance of having stroke (OR: 0.58 95% CI:0.34-0.99 ) while those with the highest intake of high-fat dairy had 128% greater chance of it (OR: 2.28 95% CI:1.29-4.00). Further adjustment for physical activity and smoking made the association of low-fat dairy intake with stroke marginally significant (P=0.07) but just slightly attenuated the association of high-fat dairy consumption and risk of stroke (2.19 1.24-3.88). The association between high-fat dairy consumption and stroke even persisted after additional adjustments for dietary variables (OR for top vs. bottom tertile: 2.02 95% CI: 1.02-4.02). Conclusion: We found evidence indicating a significant positive association between high-fat dairy consumption and risk of stroke. Further prospective studies are required to confirm this finding.
