بررسی عوامل همراه در بهبود افتادگی پا
A Survey of the Associated Factors in Foot Drop Improvement

چکیده مقدمه: افتادگی پا مشکلی شایع و ناراحت کننده است که میتواند منجر به افتادن و صدمه بدنی شود. با توجه به نگرانی بیمار و پزشک از پیشآگهی این عارضه، بهبود نسبی در پارهای از موارد و نبود مطالعه کامل در این مورد در کشور ما، ایران با وجود انواع درمانهای انجام شده، مطالعه و بررسی دقیقتر برای ارزیابی پیش آگهی در این بیماران ضروری به نظرمی رسد. هدف: تعیین عوامل همراه در بهبود افتادگی پا با توجه به سن، جنس، نوع و علت ضایعه، زمان بروز تا ارجاع و نوع درمان مواد و روشها: این یک مطالعه توصیفی رو به جلو است که درمرکز اکترودیاگنوزیس بیمارستان پورسینای رشت در سال90-1389ا انجام شد. پس از ورود بیماران مبتلا به افتادگی پا به طرح، اطلاعات دموگرافی،علت و زمان شروع آسیب ثبت شد. بیماران تحت معاینه کامل عصبی قرار گرفتند و سپس یک نورولوژیست مجرب در زمینه الکترودیاگنوز آنها را EMG-NCV نمود. بیماران بعد از تشخیص آسیب بطور معمول مداوا شدند. هیچ دخالتی در روند درمان بیماران و نوع درمان یادداشت شد. بیماران ماهیانه ویزیت شده، بهبود وتغییرات ایجاد شده در نوع درمان ثبت شد. در صورت انصراف، بیمار از مطالعه خارج شد.شش ماه بعد با توجه به سن، جنس، نوع و علت ضایعه، زمان بروز تا ارجاع و نوع درمان کلیه بیماران از نظر بهبود بررسی آماری شد. نتایج: 105 بیمار(61مردو 44 زن) با میانگین سن 3/17± 45 سال در این مطالعه شرکت کردند. 2/56 درصد بیماران بهبود داشتند. فراوانی بهبود در ریشه عصبی 1/44%، عصب سیاتیک 75 %، عصب پرونئال مشترک 75% وعصب پرونئال عمقی 9/88% بود که از لحاظ آماری معنیدار بود (005/0>P). فراوانی بهبود ضایعات اکسونال 5/53% و ضایعات دمیلینزان 100% بود که از لحاظ آماری معنی دار بود (028/0>P). فراوانی بهبود افتادگی پا در تروما 6/75%، رادیکولوپاتی 6/38% و در کمپرسیون موضعی 7/85% بود که از لحاظ آماری معنی دار بود (0001/0>P). فراوانی بهبود بر اساس درمان انجام شده از لحاظ آماری معنیدار نبود. 95% افراد بهبود یافته میانگین سنی 35-43 سال داشتند و 95% افراد بهبود یافته در فاصله زمانی 8/0 تا 2 ماه از زمان بروز ارجاع داده شده بودند که هر دواز نظر آماری معنی دار بود (0001/0 > p). بحث و نتیجهگیری: عوامل پیش بینی کننده فراوانی بهبود، طول مدت بین بروزاختلال تا ارجاع وسن بیمار است.
Introduction: Foot drop is a common and distressing problem that can lead to falls and injury. Considering the respective Patients and physicians’ concerns regarding the prognosis of this condition, relative improvement in some cases and no complete study about this problem in our country despite various treatments attempted, a closer study to assess prognosis in these patients seems necessary. Objective: The aim of this study is to investigate the associated factors in foot drop improvement including age, gender, site, cause and type of injury, time between the incidence and referral, and type of treatment. Materials and Methods: The present study is a cross- sectional one carried out with the patients referred to electordiagnosis department of Pursina hospital of Rasht in 2010-11. After entering the patient into study, demographic information, cause and damage started time were recorded. All the patients after a complete neurological examination were assessed by a neurologist experienced in EMG-NCV. Afterwards, the vulnerable patients were treated routinely. No intervention was done in the process of treatment and recorded the type of treatment. Patients had monthly visits, with improvement and changes in treatment recorded. If a patient withdrew he was excluded. Six months later, based on the age, gender, site, cause and type of injury time between the incidence and referral and type of treatment, all the patients were compared in terms of recovery. Results: Totally, 105 patients (61 males and 44 females) with mean age of 45±17.3 years participated in this study. 56.2% of them were cured. The improvement in the root was (44.1%), sciatic nerve (75%), common peroneal nerve (75%), deep peroneal nerve (88.9%), which was statistically significant (P< 0.005). The recovery rate of axonal lesion (53.5%) and Demyelinating lesion (100%) which were statistically significant (P<0.028). The recovery rate of foot drop in trauma was (75.6%), radiculopathy (38.6%) and local compression was (85.7%) which was also statistically significant (P<0.0001). The recovery rate of foot drop according to type of treatment was not statistically significant in 95% of improved patients, the average age was 35-43 years (P<0.0001) and time between the incidence and referral in 95% of improved patient, were within 0.8 to 2 months, which was statistically significant (P<0.0001) Conclusion: Predictive factors of recovery were age of the patient and time between the incidence and referral.
