Post-Stroke Seizure and Its Risk Factors

Introduction: Seizure is one of the complications of stroke that occurs early and late after stroke. If it leads to brain hypoxia, it aggrevates the patient’s condition. Determination of its frequency and risk factors helps predict its occurance and respective precaution in stroke patients
Objective: Determination of the frequency and risk factors of post-stroke seizure.
Materials and Methods: This comparitive cross-sectional study was carried out on 1256 stroke patients’ hospitalizedin neurology ward in Poursina hospital in Rasht. Data were collected using designed questionnaire from files of patiens or by telephone interview with patints, families or if nessecary by calling them to be visited in neurology clinic of Guilan University of Medical Sciences. Data were analyzed by independent-T and Qi square tests and logistic regression model in SPSS software version 21. p<0.05 was considered as the significance level.
Results: Among the studied patients, 46 patients (3.29%) with mean age of 62.8±11 yearshad seizure. 69.6% and 30.4% of patients had early and late siezures, respectively. Cerebral embolism (p=0.032), history of stroke (p=0.008) and family history of stroke (p= 0.016) were determined as three predictors associated with post stroke seizure.
Conclusion: The prevalence of post-stroke seizure is relatively low. Cerebral embolism, history of CVA and family history of CVA are as the predictors of post stroke seizure.

The Relationship Between miR-423 Genetic Variation and Women Breast Cancer Risk In the North of Iran

Introduction: MicroRNAs (miRs) are small noncoding RNAs that inhibit protein expression by posttranscriptional inhibition. They are fundamental regulators of diverse cellular processes; whose deregulation contributes to many human diseases including cancer. Single nucleotide polymorphisms (SNPs) of the genes encoding miRNAs can alter miRNA expression and may influence cancer risk.
Objective: To determine the relationship between miR-423 (rs6505162 C>A) polymorphism and women breast cancer risk.
Materials and Methods: In this case-control study, the genotypes of 353 women with breast cancer and 353 healthy women were compared. Genomic DNA was extracted from peripheral blood. The miR-423 rs6505162 C>A polymorphism was determined by PCR-RFLP using RsaI enzyme. The data analysis was performed using the MedCalc software.
Results: The prevalence rates of genotype frequencies of the CC, CA and AA were 60.34%, 35.41% and 4.24%, respectively, in those with breast cancer, whilst in controls were 51%, 38.81% and 10.20%, respectively. The AA genotype and A allele of rs6505162 C>A had a decreased risk to develop breast cancer (OR=0.35; 95%CI 0.18-0.66; P=0.001; OR=0.66; 95%CI 0.52-0.85; P=0.00, respectively).
Conclusion: our data demonstrated that miR-423 rs6505162 should be a protective factor for breast cancer. Further studies involving different ethnicities are required to verify our conclusion.

The Relationship Between Vitamin-D Level and Fibromyalgia

Introduction: Role of Vitamin-D deficiency in musculoskeletal pain had been recognized previously. However, there is still controversy about the relationship of fibromyalgia, as a prototype of musculoskeletal pain disorders, with Vitamin-D deficiency.
Objective: This study was designed to assess serum vitamin D status (25-OHVit.D) in the fibromyalgia (FM) patients and to compare it with a healthy control group. It also aimed to investigate the correlation of serum vitamin D level with FM symptom severity and invalidation experiences.
Materials and Methods: This cross sectional study was done in patients in Razi hospital, Rasht, Iran. A total of 74 consecutive patients with FM and 68 healthy control participants were enrolled. The eligible FM patients completed the Illness Invalidation Inventory (3*I), the Revised Fibromyalgia Impact Questionnaire (FIQR) and a short-form health survey (SF-12). Venous blood samples were collected from all participants to evaluate serum 25-OHVit.D levels. Mann-Whitney tests and multiple logistic regression analyses were performed and Spearman’s correlations were calculated.
Results: 88.4% of FM patients had low levels of serum 25-OHVit.D (below than 20ng/ml). FM patients had a significantly higher level of serum 25-OHVit.D than that in the control group (17.24±13.50 and 9.91±6.47ng/ml, respectively, p=0.0001). There were no significant correlations between serum 25-OHVit.D levels and the clinical measures of disease impact, invalidation dimensions, and health status. Multiple logistic regression analyses revealed that an increased discounting of the disease by the patient’s spouse was associated with a 4-fold increased risk for vitamin D deficiency (OR=4.36; 95% CI, 0.95−19.87, p=0.05).
Conclusion: This study showed that although high rates of vitamin D insufficiency or deficiency were seen among FM patients and healthy non-FM participants, but it seems there was no intrinsic association between FM and vitamin D deficiency. Addressing of invalidation experience especially by the patient’s spouse is important in the management of FM. Comparing the Relationship Between Self Efficacy and Marital Satisfaction In Polycystic Ovary Syndrome and Healthy Female Cases

Introduction: Polycystic ovary syndrome(PCOS) is one of the most common endocrine disorders that whose prevalence has been reported from 4 to 25 percent among women of reproductive age.
Objective: PCOS is associated with different metabolic, reproductive and psychological consequences that demands research studies. The aim of this study was to investigate the relationship between self efficacy and marital satisfaction and compare it in women with polycystic ovary syndrome and healthy counterparts.
Materials and Methods: This study was done using a causal-comparative method on 129 women with polycystic ovary syndrome and 125 healthy women. Available sampling method was employed using Rotterdam criteria. Women in both subject and control groups responded to the DAS-32 and GSE – 10 questionnaires. Data were then analyzed with SPSS software version 20 using statistical methods including pearson’s coefficient , regression, T-test and multivariate analysis of variance
Results: According to the results, marital adjustment was significantly lower in subject group than in healthy group (p<0.001). The observed difference in self efficacy between subject and healthy groups was not statistically significant, also there was a positive correlation between self efficacy and marital adjustment in PCOS and healthy groups and self efficacy could predict marital adjustment.
Conclusion: Considering the marital adjustment and its relationship with PCOS in both groups, it seems essential to increase awareness, screening and referring process in order to take advantage of the advisory services.

The Relationship of Hopefulness and Hardiness With General Health In Women With Breast Cancer

Introduction: Some chronic diseases like cancer, and their associated side effects are considered as stressful life events. But, these reactions are not necessarily the results of the mentioned conditions and depend on the pressure compensatory factors, like hopefulness and Psychological Hardiness.
Objective: The aim of this study is to determine the relationship of hopefulness and Psychological Hardiness with general health of women suffering from breast cancer.
Materials and Methods: This descriptive correlational study was conducted in Rasht in 2016. The study population included 108 patients of which 85 people were selected using the Morgan table. Data were obtained through questionnaires, hwMiller,s hopefulness to life, public health and psychological hardiness and analyzed using the multivariable regression statistical methods and Pearson correlation.
Results: The results show a positive relationship between Hardiness to public health and the dimensions including physical signs(p<0.01), anxiety(p<0.01), social conflict(p<0.01) and depression (p<0.01). In addition, a negative correlation was observed between hopefulness to life and public health dimensions including physical signs(p<0.01), anxiety(p<0.01), social conflict (p<0.01) and depression (p<0.01).
Conclusion: The results of this investigation can serve as the guidance to flourish psychological hardiness and hopefulness to life sense in the medical treatment of ill women.

Synthesis of Zinc Oxide-Kaolin Nanocomposite and Its Efficiency In the Photocatalytic Removal of Phosphate From Aqueous Solutions In the Presence of Visible Light Irradiation

Introduction: The amount of phosphate in wastewater, lakes and seas are progressively on the rise. Since this nutrient is an essential macromolecule and a limiting factor for the growth of algae and cyanobacteria, thus, it could be a major factor in the occurrence of eutrophication.
Objective: The purpose of this study was to synthesize zinc oxide nanocomposite coating on the kaolin and determine its efficiency in photocatalytic removal of phosphate from aqueous environments in the presence of visible light.
Materials and Methods: ZnO-Kaolin was synthesized by a co-precipitation method and the effects of some parameters such as initial pH, initial nanocomposite dosage, initial phosphate concentration, hydrogen peroxide concentration, the presence of oxygen and nitrogen gases, and organic compounds on the removal of phosphate by ZnO-Kaolin/ visible light were investigated.
Results: FT-IR and SEM analyses demonstrated favorable immobilization of ZnO onto Kaolin. The maximum removal efficiency of photocatalytic was attained in pH 3, initial concentration of phosphate 20 mgL-1 and nanocomposite dosage 0.5 gL-1 in 120 minutes. With increasing initial phosphate concentrations from 10 to 100 mgL-1, the reaction rate constant (kobs) decreased from 0.0604 to 0.0021 min-1 and electrical energy per order (EEo) increased from 158.940 to 4571.428 kWh.m-3. The efficiency of the zinc oxide – kaolin/ visible light process for reduction of phosphate was about 99.27%.
Conclusion: According to the carried out tests, it was found that zinc oxide-kaolin/ visible light can be considered as an effective method for removing phosphate with high performance in a short time.

Effects of Pure Flavonoid of Medlar Leaves on Passive Avoidance Learning and Memory In Alzheimer Model of Ovariectomized Rats

Introduction: Ovariectomy and estrogen removal by inducing metabolic syndrome reduce learning and memory and increase the likelihood of developing Alzheimer’s disease(AD). AD is a progressive neurological disorder with no definite cure. Polyphenols have shown important effects in preventing neurological disorders, due to antioxidant properties.
Objective: The goal of this study was to investigate the effect of pure flavonoid of medlar leaves, on STZ-induced memory impairment in ovariectomized rats.
Materials and Methods: In this experimental study, forty eight female Wistar rats weighing 200-250 g, were divided into six groups including: SHAM, OVX, OVX+STZ(3mg/kg), OVX+SAL, OVX+STZ+FLA, OVX+STZ +SAL. At the initial of the experiment, rats were ovariectomized, and after three weeks they were cannulated using stereotaxic apparatus. Intracerebroventricular injection of STZ(3mg/kg,2times) was used to induce memory impairment. Rats received pure flavonoid(10mg/kg, Intraperitoneal), in the STZ injection day for 21 days. All rats were tested for passive avoidance learning and memory in the shuttle box. Latency to enter to dark compartment and total time spent in that compartment were used as passive avoidance memory indices.
Results: There was no significant difference in the acquisition and retrieval of memory between OVX ,SHAM groups(p=0.873). ICV injection of STZ significantly decreased latency to enter to dark compartment and increased total time spent in that compartment(p=0.001). The OVX+STZ+FLA group receiving pure flavonoid of medlar showed a significant improvement in passive avoidance learning and memory, compared with OVX+STZ+SAL group(P=0.001( .
Conclusion: ICV injection of STZ disturbs passive avoidance learning and memory and treatment with pure flavonoid of medlar improves memory impairment induced by ICV injection of STZ.

Introduction: Psychological treatments include those treatments that can reduce the effects of diabetes along with drug interventions. Of course, the use of these therapies requires a lot of empirical support. The purpose of this study was to determine the effectiveness of group therapy based on acceptance and commitment on psychological stress, loneliness and blood glucose in diabetic patients with type 1 and type 2 diabetes in Rasht.
Objective: Determination of Effectiveness of Acceptance and Commitment Group Therapy on Psychological Stress, Loneliness and Blood Glucose in Patients with Diabetes
Materials and Methods: This applied research is a semi-experimental type with pre-test and post-test design with control group. The study population consisted of all adults with type 1 and type 2 diabetes in Rasht city who were referred to hospitals and related treatment centers in the first quarter of 1396 S.H (2017). First, 100 patients filled perceived anxiety questionnaires of Cohen et al (1983) and loneliness questionnaires of Russell et al. (1996), and their blood glucose were measured by glycosylated hemoglobin test. Of them, 30 were purposefully and homogeneously selected from the other features and randomly placed in the experimental and control groups. The experimental group received 8 training sessions of ACT with 90 minutes duration and then the relevant data were collected again. Data were analyzed by multivariate analysis of covariance test to investigate the hypotheses.
Results: The findings of the MANCOVA test showed that group therapy based on acceptance and commitment improved psychological stress and loneliness of the experimental group and did not reveal a significant difference in blood glucose.
Conclusion: The findings of this study emphasize the effectiveness of group therapy based on acceptance and commitment to reduce psychological stress and loneliness in people with diabetes.
