Odia ABSTRACT Most of physiological effects of bradykinin is due to its effect on bradykinin B2 receptors. There is no reports of preparation of a photoactivatable analogue of bradykinin reactive with sloubilized bradykinin B2 recpetors. Photoactivatable radioactive ligands are powerful tools in purification of receptors. In this study we report preparation of a photoactivatable iodinatable analogue of bradykinin reactive with soluble bradykinin B2 receptors. For conjugation with lysyl-bradykinin (LBK) a photoactivatable compound sulfosuccimidyl 2-(pazidosalicylamido ethyl-1,3-dithiopropionate (SASD) was used. Eigtheen hours after incubation in safe light(25 watt red light) LBK-SASD cojugate was prepared. HPLC experiments showed that retention time for conjugate on the HPLC column was 26 minutes. This was different from retention time of starting materials (for LBK 17 and for SASD 24 minutes). Also inhibition studies showed that LBK-SASD conjugate is capable of inhibiting binding of an antibody against bradykinin to ELISA plates coated with LBK-glut-BSA.This shows that in comparison with SASD and LBK the conjugate retained its immunoreactivity. Soluble binding assay showed that the LBK-SASD conjugate is reactive with solubilized bovine uterin myometrium bradykinin B2 receptors. With the addition of I125 to the conjugate it is possible to radiolabel the receptors. In the first step radioactive conjugate is reacted with the solubilized receptor, then upon exposure of the light a covalent bond is formed between receptor and the ligand. With monitoring the radioactivity it will be possible to check the success of the purification procedures. Marjaani ABSTRACT one of the best index to study the mother's Iron level is to measure the serum ferritin. The aim of this study is to find out the relationship between the serum ferritin in mothers and newborns, and therefore we use these indexes to assess the Iron depletion enemia in above subjects. In this study, hemoglobin, serum ferritin, transferrin saturation (%), serum iron, and total iron binding capacity were measured in blood from 52 mothers approximately one hour before delivery and in cord blood from 52 newborns after the birth. This is a cross-sectional descriptive study. To measure the serum ferritin and hemoglobin, we used ELISA and Cyano-methemoglobin method respectively, also to measure Iron and TIBC the routine Kit's Laboratory is used. For analytical study the linear reggresion analyse has been applied. Mathernal serum ferritin levels were 12.9 ± 8.9 ng/ml. This value of ferritin was lower than those of healthy (100 ng/ml) women. Mean serum ferritin level in blood newborn was 103±54.1 ng/ml and this was higher than adults. We observed an important positive correlation between serum ferritin values in maternal and newborn blood hemoglobin values. The results of this study showed, those mothers that had'nt get Iron storages, their newborn's serum ferritin level analytically is variable. Hajia ABSTRACT Recently, PCR is being reported more frequently with satisfactory results on the diagnosis of clinical infections. Widespread availability of PCR promises a sensitive and specific alternative, to traditional methods, but these benefits must be balanced against cost. Protocols using small volumes have described where the reaction taken place in capillary tubes, and small volumes work when specialized thermocycler used. Despite its benefits on increasing the speed and decreasing the cost, this technique needs special equipment and thermocycler to perform which may not be possible for most molecular diagnostic laboratories. In this study, we wanted to evaluate effects of reducing time and volumes of reagents and therefore increasing speed and lowering reagent costs by routine equipment and ordinary thermocycler. In this study, M. pneumoniae DNA was extracted by phenol Chloroform method after treating with proteinase K. PCR was performed in different volumes and various amplification, and PCR products were analyzed by gel agarose electrophoresis. Experiments performed with extracted M. pneumoniae DNA confirmed that volume of reaction mixture can be decreased from 100 to 10 ul. Obtained results also proved. amplification time can be decreased from 6.5 to 3.5 minute for each cycle without losing of the sensitivity. These finding indicate dropping of the cost up to one tenth and necessary amplification time to the half in comparison with conditions. In the next step 34 throat swabs were tested with both conditions. All specimens had same sensitivity for optimized and main conditions. Performing PCR in small volumes not only reduce the cost of the test, but also can remarkably increase speed of the DNA amplification. Aliee ABSTRACT Evaluation of intrauterine growth and determination of at risk neonates was done by weight criteria for each pregnancy age. This criteria in different populations is not similar. Main objective of this study was determination of intrauterine growth pattern in this region. Weights of 2380 term neonates and 788 preterm neonates that had a definite LMP and were born at Alzahra hospital in 1997 were used in this study. Curves of 5th, 10th, 50th, 90th and 95th percentiles was created. Medians were compared for comparison of curves. Our curves was Lower than U.S.A. neonates curves (P<0.01). Curves of boys and multipar mothers were higher that girls and primpars respectively(P<0.01). Haashemi ABSTRACT The epidemiological studies indicate that the incidence of salmonellosis is increasing throughout the world. The uncontroled and unappropriate usage of antibiotics has been caused multi - drug resistance in these organisms, in recent years. In a cross-sectional descriptive study, 204 strains of typhoidal salmonella (T.S) and 114 strains of non-typhoidal salmonella (N.T.S) were examined to determine drug resistance. The strains were collected from patients who referred to clinical centers in Hamadan during 1994 to 1997. They were serotyped and then tested for their antibiotic resistance patterns, using Kirby-Bauer method for 8 antibiotics. The salmonella isolated were as follows: "S.typhi, S. paratyphi A, B, C, S.murium, enteritidis, S. choleraesuis, S. agona, S. arizona, S. infantis, S. havana, S. lexington and S. virchow." A proportion of strains (> 60%) were resistance to Carbenicillin and ampicillin. Resistance to Ciprofloxacin and Nalidixic acid was very low (< 15%). S.typhimurium (100%), S. typhi (95.7%), parathyphi B (89.2%) and enteitidis (60%) showed multi-drug resistance (MDR). We suggest that the use of some newer antibiotics such as new fluoroquinolones, ceftazidime, imipenem and aztreonam as effective therapy againts salmonella species. Asefzadeh ABSTRACT Inappropriate antibutic prescription cause both microbic resistance and imposition of heavy and useless expenditure on patient and remedy-drug system. To analyse prescription indexes regarding antibutics in Qazvin physicians' prescriptions, this descriptive study was conducted on 2100 regular, randomly sampled prescriptions of general physicians, specialists and dentists. These documents were obtained by referring to Medical Document Registry of Drug Service Organization in 1998. The findings indicated that 43/2% of prescriptions contained antibutics; 51/8% in General physicians' 32/2% in specialists' and 93% in dentists' presciptions. ENT spesialists had the most antibutic prescription (62/5%), afterward places pediatrics (58/6%). The most common used prescriptive antibutic was Amoxiciline (23/3%), and also we noted that 30/7% of total antibutics were injective. Excessive and illogical drug prescription, especially antibutics, which might have serious consequences, could originate from weak teaching prescription writing. Ajmi B . Hagh panaah ABSTRACT Toxoplasma gondii is an obligate intracellular parasite which is capable of infecting most of the hot blooded vertebrates. This parasite has a very different prevalence rate. Enviromental conditions and patern of food consumption are effective factors on prevalence rate of infection. Due to an appropriate geographical conditions , Mazandaran province has been one of the most important areas on the basis of infection with T. gondii. This study was conducted on butchers of Sari, because the prevalence rate of infection in butchers seems to be high. The present survey which is a discriptive study, was done on all permanent workers of butchery. Blood samples of all cases were collected, and to determine the antibody titer , indirect immunofluorescence test was used. Also a questionnaire was filled up by them on the history of previous infection, contact with cats and their nutritional habbits. Results show that 87.8% of the examined butchers has a history of infection with above mentioned parasite, 70. 1% had a habbit of eating a semi-cooked meat, and 8% had a history of contact with cats in their living places. Taking into consideration, the appropriate geographical conditions and high infection rate in Sari, the health education for correction of nutritional habbits and also hand washing after handling of meats by butchers are advised in these high risk cases as a measure of control. All of serum sampels were examined by indirect immunoflouroscence antibody (IFA) method. Difference of antibody titer between case and control group was significant (P<0/01). In duration of work at the slaughterhouse and age of workers wasn't any significant difference, but there was singificant difference in worker's antibody titer between variuse parts of slaughterhouse as work place (P<0.01). Sayyah ABSTRACT Every year in the city of Kashan various number of cases of malaria has been observed. Determination of the prevalence of the disease and the carriers in this area of the country can play a significant role in employing preventive measure as well as in managing logistic services. Through applying retrospective descriptive methods and based on the statistics reported from the Center of Communicable Disease in Kashan, we found 498 cases of disease reported to the health centers from Farwardin 1365 to Aban 1376, and a type of species of malaria parasite were detected in their blood smear. From the total number of cases, 85% of those identified as positive were Afghan immigrants, and 5% were other Iranian immigrants who came to Kashan from the infected areas. 91% of the cases were male and 9% were female. 77.7% of the malaria cases were identified as being infected by P. vivax, 16.1% by P. falsiparum, 6% by Mix infection, and one case by P. malariae. The age group 20-29 included the majority of the cases (46.4%) The least infected agegroup belonged to the age 50 and over (3%). The range of infection for age included a 2year old boy to a 70-year old woman. Four cases of recurring in patients infected by P. vivax was observed. Based on entomological studies in this city, at least three type of anopheline mosquitoes, An. claviger, An. multiculor and An. superpictus, have been identified. An. superpictus that is identified as a carrier in Iran, is active in some locations of this city from Ordibehesht to Aban. The distribution of larvae and adult An. superpictus in the area exceeds the distribution of any other types. The adult of this type of anopheline mosquito has been captured in both human and animal residing places in Khordad and Mehr. The available statistics, indicates that malaria at the present time is not a serious health problem, but mean while, considering the availablitiy the vector chains, malaria in this city,must be under close scrutiny, cases must be identified, treatment must be used, and more serious entomology check and spraying poison of breeding sites than before must be done. Mohammadzadeh ABSTRACT Atherosclerosis is one of the most common disorders in Iran. It usually involved adult males over the age of 60 year. All Major and middle arteries are at risk and usually it has two types effects on arteries, Occlusive lesions in the site of dividing arteries and aneurysmal lesions which occure in abdominal aorta in 90%. Frequency, symptoms and pathologies of atherosclosis is still unknown in Iran and hadn't studied in Guilan yet. For clarify these vagues, records of 112 patients who was underwent surgery for effects of Atherosclerosis was reviewed retrospectively. There were 96 males and 16 female. Range of age was 42-78 year with main 57±3 for males and 63±4 for femals. 26% has diabetes and Atherosclerasis. 106 had history of smoking for a long time. 75 (68%) had occlusive lesions and 37(32%) Atrial Aneurism. In 17 patients Abdominal aortal Aneurism was diagnosed by chance in daily visits by medical proficents. 24 Artifical Dacran Grafts and 13 Grafts with Saphenous vein was used for other peripheral aneurisms. 1 Case death due to acute myocardial infraction and 1 case left femoral Artery embolism was occured after Aneurysmectomy. This survey showed that abdominal aortal aneurism is relatively common but often undiagnosed. In advanced stages pulsatile mass over 60cm without any symptom is the only diagnostic finding in 55% patients. Dadashi ABSTRACT Pleural cffusion is caused by many disease, and also is a symptom of others. There are manys for diagnosis of causes of pleural effusion, such as thoracocentesis and needle biopsy. In about 15 to 20 percent we couldnot find any specific causes, for pleural effusion, and we need more Invasive procedure for definitic diagnosis. such as thoracotomy or thoracoscopy. Today most surgcons rarely use Thoracotomy as the first line invasive procedure for finding the cause of pleural effusion today. Thoracospic evaluation now is more popular. The main goal of this stady is to present the superiority of thoracoscopy than thoracotomy for extensive evaluation of the cause of pleural effusion's. Because of less complication, and more diagnostic Insight and less need thoracotomy. In this study, 40 patients, that had pleural effusion, and we couldnot find the cause of pleural effusion, by Aspiration and necdle biopsy, and thoracopic exam, was performed. In regard to sex of the patients, 28(70%) was male and 12(30%) was female. The mean age was 30 years old, and upper and lower limit was 10-70 years old. Primary diagnosis, was according to clinical finding and 20(50%) patients has pleural cffusion with unknown cause. 8(20%) patients had TB, 6(10%) patients had Empyema and 2(5%) patients had mesothlioma. Thoracopic finding that guide for some disease was: 1 Fluid Aspiiration and direct pleural biopsy in 10 patient 2. Diagnosis of pleural adhesion with fluid aspiration and direct pleural biopsy in 8 patients, 3. TB In 8 Patients. 4.Empyema in 6 patients, 5. Pulmonary metastasis in 4 patients, 6. Mesothelioma in 2 patients,7.Lung mass in 2 patients After tissue sampling and pathologic result, Defintic diagnosis, was as below: 1. TB 28 (70%) 2. Adenocarcinoma metastatic 4(10%) 3. Mesothelioma 3(7.5%) 4. Askin Tumor 2(5%) 5. Squamus cell carcinoma (SCC) of lung 2 (5%) 39(97/5) patients with this mode of evaluation has definitic diagnosis of cause pleural effusion And only in one case thoracotomy was the way for diagnosis. Complications of thoracoscopy was Air leak in 4 (10%), patient's wound infection in 2 (5%) cases with no mortality. Therefore we suggest thoracospic evaluation as The first line for definitic diagnosis, which is the best way for diagnosis before thoracotomy. Karami far ABSTRACT There are different causes for rickets. Vitamin D deficiency, disorder in vitamin D metabolism and action, familial x.linked hypophosphatemia, renal failure, hepatic disease & oncogenous rickets are some of the major causes of rickets. In this research we had the opportunity to study 50 cases of rickets during one year period in shiraz. The results showed that vitamin D deficiency was the most common form of rickets. 36 cases (72%) had vitamin D deficiency rickets, 3 cases (6%) had vitamin D dependent rickets and 11 cases (22%) had vitamin D resistant rickets. The sex distribution was (52%) females and 24 (48%) malcs. There was not a significant difference in female to male ratio in this study. We found that 71.4% of patients had used breast milk during infancy and they had not vitamin D supplement. Almost all of them were from cold climate regions of Fars province. Sixteen (32%) of vitamin D deficiency cases were under one year age and they were cared at home. Radiological finding in wrist x. Ray was persent in all patients. The most important symptoms of patients that coming to hospital or office were convulsion and infections especially pneumonia. These results indicate that although several factors are concerned in the development of rickets, the main cause is lack of vit D. Saharkhiz ABSTRACT Despite vasectomy is one of the surest contraceptive methods with minimum complication, has been recieved reluctantly by our society. One of the various factors of this is men's knowledge deficiency about salty of vasectomy. The aim of this study is survey of the men's attitude who was underwent vasectomy after a relatively long period of their operation. This could help men who are in doubt for choosing vasectomy as a superior alternative for prevention. In this descriptive study we selected 400 men who was underwent vasectomy in Professional and Investigative Center of Family Planning in Guilan three years ago and was premanent resident of Rasht and suburb. Data collected by a questionnaire about factors influence their attitude about vasectomy. 300 (75%) of cases was reside in Rasht and 100 (25%) in suburb. 388 (97%) of cases believed that the information that they recieved before is concident with realites, 372 (93%) valued vasectomy as a better method than other contraceptive methods, 10 (2/5%) regarded vasectomy as a worse method in comparison with the methods they used before, 18 (4.8%) saw no difference between vasectomy and the method they used formerly. 398(99.5%) advised vasectomy to others. The results of this survey showed that three years after vasectomy the attitude of them was positive. So vasectomy is an ideal permanent procedure for contraception and we can advise it with surely. Kalantary ABSTRACT The prevalence of hypothyroidism & mental developmental disturbances has been shown to be increased in iodine deficient arease. To Clarify the extent of this relation in Iran, we elected to survey 346 mentally retarded patients of Gillan Institudes. Aster filling a check list for every patient, physical examination was done & blood sample was kept for thyroid function (T3, T4, TSH Ib) measurment with commercial kits. In Patients with hypothyroidism, Thyroid scan & X. Ray of wrist for bone age was done. The Prevalence of goiter was 25% (84 patients), goiter grade I, II & III was detected in 54 (65%), 16 (19%) and 4 patients (5%) respectively. T4 concentration in 6 patients (1.7%) was reported to be lower than normal, T4 was between 2.1-3.1 mg/dl in 2 cases & 4 patients had T4 lower than 2 mg/dl. 17 patients has TSH concentration more than 5 mu/L with a majority of TSHs (59%) between 5-10 mu/L. TSH was more than 10mu/L in 7(2%) patients. 4 patients had secondary hypothyroidism so taking them into account, the prevalence of hypothyroidism was 6% (21 cases). 2 out of These 21 cases (0.57%) had congenital hypothyroidism. 9.8% (34 patients) of mental retards had Down syndrom & 5 of them (15%) were hypothyroid. The increased prevalence of hypothyroidism in this survey emphasize the necessity of neonatal screening for hypothyroidism. Mahjub ABSTRACT Health screening in student, such as Visual screening test usually carry out by school nurses in many rural schools in Iran. While students are in the growth age, visual acuity may be affected by growth. A Study was carried out to evaluate vision screening test on students in Hamadan, Iran. A sample of 878 pupils examined by a school nurse using E- Chart. According to the test, children were categorized in two groups of negative and positive. All children having disorder in vision were categorized as positive and a sample of others were refered to an optometrist for more examination and diagnosis. According to the screening test and diagnosis, children were categorized in four groups; true positive, false positive, true negative and false negative. Sensitivity, specificity, positive predictive value and negative predictive value calculated for the data. The results showed that the sensitivity, specificity, positive predictive value and negative predictive value for disorder in vision was 44.8%, 98.9%, 87.5% and 91.5%, respectively. In a comparison study, sensitivity and specificity increased with ignore of hypermetropia students in the data. In the case, sensitivity and specificity are 68.2% and 99%, respectively. This survey showed low sensitivity E- chart in diagnosis of hypermetropia. It can be concluded that if students having reduced visual acuity by the test, refere to an optometrist is recommended. R Ghaffari Nejad ABSTRACT Many medical patients is general hospital suffer from depression and it seems that depression correlated with longer hospital stay, so correct diagnosis of depression and early pshychiatric consultation are important matter of cocern. In this research prevalence of depression and psychiatric consultation for depressed patients were studied in one of three general hospital of Kerman University of Medical Sciences. 508 medical in patients who stayed at least one week in hospital were studied by Beck Depression Inventory (BDI), also their demographic factors such as sex, age and hospital ward were studied. After discharge depressed patient's charts were reviewed to see if psychiatric consultation was done or not. Prevalence of depression was 59/1% and prevalence of depression in women's internal ward was higher than other wards (X2= 19/79, P= 0/0049). prevalence of depression was higher in signles and women than married and men respectively. Psychiatric consultation, were requested for only 25 depressed patients (% 8/3). Incomplete knewledge about effect of deppression in medical disorder's course and absence of psychiatric ward in general hospitals may explanatory.
