Objectives: Antibiotics and analgesics are commonly used in dentistry to treat or prevent infections and manage pain. However, their misuse can lead to significant adverse effects for both individuals and communities. This study aimed to assess the level of knowledge among general dentists in Guilan (north of Iran) in 2023 regarding the principles of antibiotic and analgesic prescription for root canal therapy.
Methods: This cross-sectional analytical study involved 153 general dentists who completed a self-administered questionnaire, developed based on reliable sources, consisting of 3 demographic questions and 28 clinical questions. Statistical significance was set at 0.05 in IBM SPSS V23. 
Results: A total of 153 dentists (108 males and 45 females) responded to all questionnaire items. Dentists' knowledge was categorized as follows: 19.6% moderate, 19.6% good, and 60.8% excellent. Amoxicillin was the most commonly prescribed systemic antibiotic for periapical lesions, while metronidazole and clindamycin were the most frequently prescribed for penicillin-allergic patients. Additionally, over 70% of participants demonstrated a good understanding of the principles and dosages of analgesic prescription. 
Conclusion: The results indicate a high level of knowledge among general dentists in Guilan. However, the study clearly showed that dentists with 5 to 10 years of clinical experience required structured and effective programs to update their knowledge.
