Background: The underlying causes of medical errors and complaints against radiologists depend on several factors, which can be referred to lack of sufficient knowledge, incorrect judgment, weak and inefficient techniques. In order to prevent the repetition of these errors, it is necessary for radiologists to report the errors that have occurred.
Objective: It is hoped that with the results of this study and knowledge and awareness of the most important reasons for complaints against radiologists, it is possible to take an effective step towards reducing complaints and improving the quality of services provided to patients.
Methods: This study is a retrospective cross-sectional descriptive study that was conducted using available data or information registered in the General Department of Forensic Medicine.
Results: During the 5-year study period, a total of 9 complaints against radiologists were registered in Guilan province, were from private centers. 66.7% of the patients were under the age of ten  and 77.8% of cases were male. In 77.8% of cases, errors in observations were identified. In none of the cases was the radiologist found guilty, and the final outcome of the investigation was acquittal.
Conclusion: Based on the results of this study, it was shown that the number of complaints registered from the radiology department is very limited.  All of cases were from private centers and in none of the cases was the radiologist found guilty and the final result of the investigation was acquittal.
