بررسی ارتباط جریان خون جانبی قلبی با پیامدهای بالینی در بیماران دچار نشانگان کرونری حاد

چکیده مقدمه: امروزه اهمیت گردش خون جانبی و عوامل تشخیصی آن، توجه بسیاری را به خود جلب نموده است. حضور این رگها تغییر چشمگیری در بروز آنژین، انفارکتوس میوکارد و اختلال دیوارهای بطنی یا همودینامیک ایجاد نمیکند. از سوی دیگر در گروهی از مطالعات، رگهای جانبی را عامل مشاهده الکتروکاردیوگرام طبیعی در بیماران دچار بیماری رگهای کرونر (CAD) میدانند. با توجه به اثر رگهای جانبی و وجود عدم توافق، به بررسی تاثیر گردش خون جانبی بر پیامد بالینی در گروهی از بیماران با تشخیصACS ,پرداخته شدهاست. هدف: تعیین ارتباط جریان خون جانبی قلبی بر پیامد بالینی در بیماران دچار نشانگان کرونری حاد مراجعهکننده به بیمارستان حشمت رشت مواد و روشها: در این مطالعه توصیفی- تحلیلی 99 بیمار دچار نشانگان کرونری حادکه به بیمارستان حشمت رشت مراجعه کرده بودند وارد مطالعه شدند. اکوکاردیوگرافی و آنژیوگرافی کرونرو کراتینکیناز نیز در آغاز ورود و سپس 12 و 48 ساعت پس از آن سنجیده شد. بیماران با و بدون رگهای جانبی در طی یک سال از نظر پیامدهای بالینی در برگیرنده مرگ، انفارکتوس مجدد و بستری مجدد به دلیل ایسکمی تکرار شونده و کاستی جهشی سیستولی بررسی شدند. ارتباط جریان خون جانبی با پیامد بالینی براساس نوع راهبرد درمانی انتخاب شده پس از آنژیوگرافی در بیماران جداگانه بررسی شد. دادهها با استفاده از امار توصیفی و استنباطی با نرمافزار SPSS نسخه 20 تجزیه و تحلیل گردید. نتایج: از 113 نفر، 99 نفر وارد مطالعه شدند که 6/56% مرد و 4/43% زن بودند. میانگین سنی نمونهها 75/9±34/59 سال بود. در آنژیوگرافی 40 نفر (4/40%) با رگهای جانبی و 59 نفر (6/59%) بدون رگهای جانبی بودند. میان دو گروه از نظر جنس، مدت بستری، تعداد ضربان قلب در دقیقه، فشارخون سیستولی و دیاستولی و میانگین فشارخون شریانی تفاوت معنیدار دیده نشد. بین دیابت شیرین و جود رگهای جانبی تفاوت معنیدار مشاهده گردید (001/0p=). در میزان کاستی جهشی سیستولی بین دو گـروه در دو زمان ابتــدا و پس از 1سال هیــچ تفاوت معنیدار دیده نشد. همچنیـن، بین میزان حفظ عملکرد سیستولی قلب، تغییر کاستی جهشی سیستولی در عرض 1 سال در بیماران با و بدون تشکیل گردشخون جانبی ارتباطی وجود ندا شت همچنین، ارتباط معنیدار بین وجود رگهای جانبی در بیمار و دو متغیر بستری مجدد و مرگ پیدا نشد. نتیجهگیری: این پژوهش نشان داد که پیامدهایی مانند مرگومیر و بستری مجدد و حفظ عملکرد سیستولی در بیماران با تشخیص ACS (در بردارنده آنژین قلبی و MI)، بین بیماران با و بدون رگ های جانبی تفاوتی وجود ندارد
Abstract Introduction: The importance of collateral circulation in acute coronary syndrome and its diagnostic factors has attracted a lot of attention. The presence of these vessels does not make a significant change in the incidence of angina, myocardial infarction and ventricular walls or hemodynamic disorder. On the other hand, in some studies, peripheral blood vessels in patients with CAD cause a normal electrocardiogram View. Due to the effect of collateral vessels and the presence of disagreement, the effect of collateral circulation on clinical outcome in a group of patients with ACS, are discussed. Objective: Identify the relationship between coronary collateral blood flows on clinical outcomes in patients with acute coronary syndrome admitted to hospital in Rasht Materials and Methods: This cross sectional study on 99 patients with acute coronary syndrome who are referred to hospital Heshmat coronary angiography was performed. Coronary angiography, echocardiography and creatine kinase also initially and then 12 and 48 hours after it was measured. Patients with and without collateral vessels over a year in terms of clinical outcomes, including death, reinfarction and recurrent ischemia and shortcomings of readmission due to systolic ejection were included. Its peripheral blood flow with clinical outcome depending on the chosen treatment strategy in patients after coronary angiography was studied. Using descriptive and inferential statistical analysis was performed with SPSS version 20. Results: 99 from 113 patients (56.6% were male and 43.4% were female) remain in the study. Mean age of samples were 59.34±9.75. 40 ( 40.4%) of patients had collateral vessels and 59(59.6%) of patients had no collateral vessels. The difference between sexes, length of hospitalization, number of heart beats per minute, systolic and diastolic blood pressure was not significant for the presence of collateral vessels. There exists a significant relationship between diabetes mellitus and coronary collaterals. (p=0.001 ( . No significant differences have been observed in the systolic ejection fraction between the two groups at baseline and after two periods of 1 year. There is no significant correlation between neither readmission nor mortality and coronary collaterals. Conclusion: This study showed that, in patients with ACS (includes angina and MI), in outcomes such as mortality and readmission and preserved systolic function in patients with and without collateral vessels, there is no significant difference Conflict of interest: non declared
