ارتباط قندخون پیش از آنژیوگرافی و نفروپاتی ناشی از مادهی حاجب پس از آنژیوگرافی رگهای کرونر در نشانگان حاد کرونری
The Relationship Between Pre Procedural Glucose and Contrast Induced Nephropathy in the Patients with Acute Coronary Syndrome Undergoing Coronary Angiography

چکیده مقدمه: آسیب حاد کلیوی ناشی از ماده حاجب(CI-AKI) یکی از عوارض آنژیوگرافی کرونر است زیرا تجویز مادهی حاجب ممکن است سبب آسیب کلیوی شود که در بردارنده استرس اکسیداتیو، آسیب ناشی از رادیکال آزاد و اختلال کارکرد اندوتلیال میباشد. همهی این فرآیندها در زمینهی هیپرگلیسمی نیز فعال میشوند. بنابراین، ممکن است ترکیبی از هیپرگلیسمی قبل از آنژیوگرافی و قرار گرفتن در معرض مادهی حاجب بهطور چشمگیر خطر CI-AKI را افزایش دهد. هدف: بررسی آسیب حاد کلیوی ناشی از ماده حاجب پس از آنژیوگرافی در بیماران دیابتی و غیردیابتی با نشانگان حاد کرونری مراجعهکننده به مرکز قلب حشمت مواد و روشها: همهی بیمارانی که با تشخیص نشانگان حاد کرونر در بیمارستان بستری و مورد آنژیوگرافی عروق کرونر قرار گرفتند وارد مطالعه شدند. میزان سرمی کراتینین و قندخون به همراه سایر آزمایشهای معمول در مرحله حاد و 24 ساعت و 7 روز پس از آنژیوگرافی بررسی شدند. نتایج: آنالیز چندگانه عوامل پیشبینیکننده ارتباط میزان قندخون پیش از آنژیوگرافی و نفروپاتی ناشی از ماده حاجب پس از آنژیوگرافی با کنترل متغیرهای مداخلهگر و بیماری زمینهای براساس مدل لوجستیک رگرسیون نشان داده شد که در بیماران دیابتی میزان قندخون قبل از آنژیوگرافی(32/0p=) پیشبینیکننده نفروپاتی ناشی از ماده حاجب است. نتیجهگیری: افزایش میزان قندخون قبل از آنژیوگرافی پیشبینیکننده نفروپاتی ناشی از تزریق ماده حاجب در افراد دیابتی تلقی شده پیشگیری از آسیب حاد کلیوی ناشی از ماده حاجب و مونیتور دقیق پس از آنژیوگرافی پیشنهاد میشود.
Abstract Introduction: Contrast Induced- Acute Kidney Injury (CI-AKI) is a common complication of coronary angiography. Contrast material may cause kidney damage, including oxidative stress, free radical damage and endothelial dysfunction. All of these processes in the context of hyperglycemia also rumored to be enabled. It is therefore possible that the combination of hyperglycemia before the procedure and exposure to contrast agent during coronary angiography dramatically increases the risk of CI-AKI. Objective: Stady of The Relationship Between Pre Procedural Glucose and Contrast Induced Nephropathy in the Patients with Acute Coronary Syndrome Undergoing Coronary Angiography Material and Method: All patients who were hospitalized with a diagnosis of acute coronary syndrome and coronary angiography were enrolled. Serum creatinine and blood sugar along with other routine tests in the acute phase, when the patient is admitted to hospital, were recorded. 24 hours and 7 days after angiography creatinine level was measured again. Results: Multivariate analysis predictors of blood sugar level before angiography and contrast induced nephropathy after angiography and disease control confounding variables, logistic regression in final model showed that in diabetic patient blood sugar level before angiography (p= 0/032), is a predictor of contrast induced nephropathy. Conclusion: Raise of blood sugar before angiography is a predictor of contrast induced nephropathy after angiography in diabetic subjects. Prophylaxis to prevent CI-AKI then close renal monitoring after procedure are recommended.
