گزارش یک مورد لارنگومالاسی شدید، اختلال بلع و ریفلاکس گاستروازوفاژ در نوزاد
A Case Report of Severe Laryngomalcia, Swallowing Dysfunction and Gastroesophageal Reflux in a Newborn

چکیده
مقدمه: لارنگومالاسی شایعترین علت استریدور در نوزادان از بین مالاسی راههای هوایی است که می تواند همراه با ریفلاکس گاستروازوفاژ باشد. از طرفی تاکنون موارد معدودی از اختلال بلع در نوزادانی که بیماری زمینهای ندارند، گزارش شده که بعضاً همراه با لارنگومالاسی و ریفلاکس هم بوده است. هر سه این اختلالها میتوانند سبب بروز FTT و بستری مکرر نوزاد و شیرخوار شود. درمان بموقع سبب کاهش عوارض در بیماری میشود. در این مقاله یک نوزاد دچار لارنگومالاسی شدید و اختلال بلع با ریفلاکس را که نیاز به مداخله جراح پیدا کرد، معرفی میکنیم تا توجه پزشکان را به سیر بالینی این اختلالها از لحظه تولد تا ترخیص جلب کنیم.
معرفی مورد: بیمار نوزاد پسر ترم حاصل زایمان طبیعی و بدون مشکل خاص طی دوران جنینی از پدر و مادر غیرمنسوب بود که با وزن تولد 3100 گرم و آپگار مناسب بدنیا آمد که پس از تولد در اتاق زایمان دچار استریدور خفیف دمی و رترکشن زیردنده و بالای استرنوم شده، به بخش مراقبت ویژه نوزادان منتقل شد. با انتوباسیون علائم وی بهتر شد سپس، چندین بار طی 5/2 ماه اول تولد انتوبه شد. وی بهطور مکرر دچار پنومونی می شد که سرانجام با درمان ریفلاکس وضعیت ریهاش بهبود یافت. بهدلیل ناتوانی در بلع، شیر گاواژ می شد ولی بهرغم دریافت کالری کافی دچار FTT هم شد. در 5/2 ماهگی با تراکئوستومی وضعیت تنفسی وی بهبود یافت و در 3 ماهگی گاستروستومی شد. در 100 روزگی با حال عمومی خوب مرخص شد. زمان گزارش 10 ماهه و با وزن 9 کیلوگرم بود.
نتیجهگیری: اگرچه بعید است که لارنگومالاسی از بدو تولد علامتدار شود ولی باید همواره آن را به عنوان یک علت احتمالی دیسترس تنفسی از اتاق زایمان مد نظر داشت. گزارش دیسترس تنفسی از بدو تولد به همراه استریدور، ناتوانی در بلع شیر، پنومونی مکرر و FTT در نوزاد بدون بیماری زمینهای، باید شک پزشک را به وجود همزمان اختلال بلع و لارنگومالاسی برانگیزاند که هر دوی این اختلالها میتواند همراه ریفلاکس گاسترو ازوفاژ هم باشد. در صورت پاسخندادن به درمان طبی و مورد انتظار، نیاز به مداخله جراحی وجود خواهد داشت.
Abstract
Introduction: Laryngomalcia is the most common cause of stridor between airways malacia in neonates.It can be accompanied by gastroesophageal reflux. Few cases of swallowing dysfunction were reported in neonates with no underlying disorders some of them had laryngomalacia and reflux too.
All three causes can lead to neonatal or infantile FTT or recurrent hospitalization.
Timely treatment decreased complications of diseases we reported a case of a newborn that contracted severe laryngomalcia, swallowing dysfunction and gastroesophageal reflux which needed surgery, in order to attract physicians’ attention to the aspect and clinical process of these complications, from birth to discharge.
Case Report: Patient was a full term baby boy who was born by normal vaginal delivery without special problem in his fetal status. His parents were not relative. His birth weight was 3100gr and he had appropriate apgar scores, he contracted mild respiratory stridor, subcostal and upper sternal retraction after birth at the delivery room and was referred to NICU.
After intubation his signs eliminated, he was intubated several times during the first 2.5 months after birth. He got pneumonia repeatedly and his lung condition improved with reflux treatment. Because of disability in swallowing, milk was gavage for him. But in spite of enough intake calories, he contracted FTT. His respiratory condition improved with tracheostomy in 2.5 months, and gastrostomy was done in 3 months old.
He released with a good condition in 100 days old. He was 10 months and his weight was 9kg.
Conclusion: Although, it is impossible that laryngomalacia have got signs at birth but it should be concerned as probable cause of respiratory distress in labor room. A report of respiratory distress at birth in company with stridor, disability in milk ingestion, recurrent pneumonia and FTT (without co-existing disease) should get physiscians suspicious of coincident swallowing dysfunction and laryngomalacia. Both two disturbances can be accompanied by gasteroesophageal reflux too. If there was no response to expected medical management, surgical interventions should be necessary.
