بررسی یافته های رادیولوژیک در بیماران با سل ریوی فعال

<strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">چکیده </span></span></strong><br>
<strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">سل ریوی معمولا بیماری مزمن بوده و عامل آن مایکوباکتریوم توبرکلوزیس میباشد. بمنظور مبارزه با این بیماری، با هدف یافتن بیماران مبتلا به سل ریوی فعال و تطبیق یافته های رادیولوژیک با آن در مطالعهای گذشته نگر و در بررسی رادیوگرافی قفسه سینه در ۱۰۱ بیمار با اسمیر خلط مثبت از نظر باسیل اسید فست (بیماری سل فعال) در مرکز ارجاع آموزشی - درمانی رازی رشت در طی دو سال نتایج زیر بدست آمد: بیشترین یافته ها کدورت (</span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">Consolidation</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">) ( %5/46) بوده با نماهای متفاوت، بیش از همه در قله های دو ریه و نیمه فوقانی ریه راست (7/23%) که ۳ مورد از آنها نمای کاملا بهبود یافته داشتند و سپس بترتیب حفره (</span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">cavity</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">) ( %6/35) بیشتر در لبهای فوقانی ریه، بعد، تجمع مایع در حفره جنب(</span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">Pleural effusion</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">) ( %6/12) و میلیاری (9/0%) بوده است. ۴ مورد هم </span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">CXR</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;"> نرمال داشتند. موارد خلط خونی (</span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">Haemoptysis</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">) و درصد بالاتر تعداد میکروب در خلط بترتیب مربوط به وجود کدورتهای ریوی و حفره بوده است. در زنان و مردان از نظر میزان شیوع یافته های رادیولوژیک و علائم بالینی تفاوت عمده ای وجود نداشت. در تائید یافتههای قبلی در سایر منابع، در منطقه ما هم تصاویر </span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">CXR</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;"> در بیماران خلط مثبت از نظر باسیل اسید فست، می تواند در حد بسیار پیشنهاد کننده، (</span></span></strong><strong><span dir="LTR"><span style="font-size:10.0pt;">highly suggestive</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:10.0pt;">) علامت فعال بودن بیماری سل ریوی باشد.</span></span></strong>
ABSTRACT <span dir="RTL"></span><br>
Pulmonary Tuberculosis (TB), usually is a chronic disease that caused by Mycobacterium Tuberculosis. Identification of the cases of TB is the key to control of it and radiology still plays a major role in both active and passive TB diagnosis. To assess the radiological patterns of presentation of active postprimary pulmonary TB, in this review and retrospective analysis, we studied CXR<sub>s</sub>, and medical records of 101 cases with positive sputum smear for AFB seen over a period of 2 years (1997-1999) in Razi referral center in, Rasht. The most common chest radiographical findings were as follows: consolidation, with different patterns such as reticulonodular appearance air space, fibrosis and calcification (46.5%) mostly at both apexes and upper lobes with right side dominancy (23.7%) 3 of them had healed process appearances. Cavities with air-fluid levels (mostly upper lung zones) (35.6%) and isolated pleural effusions (12.6%) and Milliary pattern (0.9%). 4 cases had normal CXR<sub>s</sub>, .Haemoptysis and high sputum bacillary burden were related in proper order to the presence of CXR consolidations and then the cavities. There were no significant differences found between sex and radiological and clinical patterns. Consisting with other reports and findings, we may consider CXR appearances in patients with AFB<sup>+</sup> sputum as "highly suggestive" for "active disease in pulmonary TB", in endemic areas.
