تأثیر دوز کم کتامین بر تغییرات همودینامیک زنان باردار حین عمل سزارین انتخابی
The Effect of Preinduction Low Dose of Intravenous Ketamine on Homodynamic Stability of Parturient Scheduled for Elective Cesarean Section

چکیده
مقدمه: ارتباط گردش خون جنین و مادر محدودیت هایی در بیهوشی عمومی برای سزارین بوجود میآورد به همین دلیل در مراحل اولیه، بیهوشی مادر عمق کافی ندارد و ممکن است با احساس درد و عوارض همودینامیک آن همراه باشد.
هدف: بررسی تأثیر دوز کم کتامین بر ثبات همودینامیک بیماران، در بیهوشی عمومی برای سزارین انتخابی.
مواد و روشها: این کارآزمایی بالینی دو سو کور بر 60 زن باردار 22 تا 38 ساله در سالهای 83-82 مراجعهکننده برای سزارین انتخابی به بیمارستان ولی عصر انجام شد. افراد در دو گروه 30 نفره قرار داده شدند. در گروه شاهد القای بیهوشی به روش استاندارد با تیوپنتال سدیم و ساکسینیل کولین و در گروه مورد (گروه کتامین) 2 دقیقه پیش از آن mg/kg 3/0کتامین وریدی تجویز شد. سپس پارامترهای همودینامیک سه دقیقه یکبار تا پایان عمل جراحی اندازهگیری و ثبت شدند.
نتایج: بعد از برش پوست، پارامترهای همودینامیک در گروه شاهد نسبت به گروه کتامین افزایش قابل توجهی داشت که از نظر آماری هم معنی دار بود(ضربان
8/18±9/119 در برابر 5/11±28/99 در دقیقه و فشار خون سیستولیک 64±2/115 در مقابل 3/9±05/109 میلی متر جیوه P<0.05) این تفاوت تا خروج جنین و کلامپ بند ناف ادامه داشت.
نتیجهگیری: به نظر میرسد تجویز دوز اندک کتامین پیش از القای بیهوشی با بهبود سطح بی هوشی و کاهش درد مادر، ثبات همودینامیک بیشتری را تأمین کند. بدین ترتیب تغییرات همودینامیک شدید اولیه در هنگام سزارین تا حدود زیادی برطرف میشوند،که فراتر از تاثیر بالینی دوز اندک کتامین است.
Abstract
Introduction: Fetomaternal circulatory communication limited liberal drug administration during general anesthesia for cesarean section surgery. This might be the cause of light anesthesia in early period of cesarean section and might induced pain and hemodynamic instability.
Objective: Survey the effect of low dose of ketamine on homodynamic stability of scheduled patients for elective cesarean section under general anesthesia.
Materials and Methods: This randomized double blinded clinical trial was performed on two equal groups including sixty 22-38 years old healthy pregnant women who scheduled for elective cesarean section in Vali-Asr hospital during 1/1/82 till 1/1/83. Anesthesia was induced with standard dose of sodium thiopental and suxamethonium and was maintained with N2O/O2gas mixture (50/50) plus 0.5 Mac halothane in both control and ketamine groups. In ketamine group, 2 minutes before induction 0.3 mg/kg ketamine, administered intravenously. The homodynamic parameters were controlled and recorded any 3 minutes in all patients until childbirth.
Results: Homodynamic parameters increased significantly after skin incision until childbirth in control group. In ketamine group there was more stable homodynamic parameters(HR 119.9±18.8 Vs 99.28±11.5, sys BP 115± 64 Vs 109.05±9.3 and p<0.05).
Conclusion: Preinduction administration of minimal dose of ketamine might produced more stable homodynamic parameters for cesarean parturient and prevented significant changes in homodynamic parameters that is beyond of clinical effect of this dose of ketamine,
