چکیده
مقدمه: سن زن میتواند یکی از عوامل خطر مهم در به مخاطره انداختن پیامد بارداری باشد. در برخی مطالعات ارتباط بین سن بالا و پائین با عوارض نامطلوب بارداری مطرح شده است.
هدف: بررسی ارتباط بین سن زن و عوارض نامطلوب بارداری.
مواد و روشها: در مطالعهای مقطعی- تحلیلی پیامد حاملگی در زنان مراجعهکننده برای زایمان به بیمارستان الزهرا(س) ارزیابی شد. متغیرها شامل فشارخون، دیابت، عوارض جفت(دکولمان و جفت سرراهی)،زایمان زودرس و پارگی کیسه آب بود که برحسب متغیر سن در سه گروه 16 سال،17 تا 34 سال و 35 ساله مقایسه و ارزیابی شدند. زنان باردار 16 سال نولی پار بوده اما دو گروه 17 تا 34 سال و 35 سال از نظر پاریتی همسان شدند. تجزیه و تحلیل نهائی با نرم افزار آماری SPSS.9 و آزمون آماری کای دو انجام شد.
نتایج: پرونده پزشکی 6045 نفر از زنان با سن حامگی بیش از20 هفته بررسی شد. از این میان 52نفر(8/0%) در گروه سنی 16 سال ،5406نفر (4/89%) 17 تا 34 سال و 587 نفر(8/9%) در گروه سنی 35 سال قرارداشتند.
یافته ها نشان داد که فشارخون القاء شده حاملگی ، دیابت حاملگی،دکولمان ، جفت سرراهی ، پارگی زودرس کیسه آب، پرزانتاسیون غیرطبیعی، زایمان زودرس و میزان سزارین در گروههای سنی 16 ، 17 تا 34 و 35 ساله اختلاف آماری معنی دار دارد( P<0/05).اما توزیع فراوانی پارگی زودرس کیسه آب برحسب این گروههای سنی ارتباط آماری معنی دار نداشت( P>0/05).
نتیجهگیری: بهنظر میرسد که حاملگی در 35 سالگی در مقایسه با نوجوانی با خطرهای خاصی همراه باشد. بنابراین آگاهی از این عوارض در برنامه ریزی برای ارائه خدمات مناسب پره ناتال نقش بسزائی خواهد داشت.
Abstract
Introduction: Age of mother at pregnancy can be one of the main risk factors for hazardous outcome of pregnancy.
Objective: The aim of this study was to determine the association between the maternal age and the outcome of pregnancy.
Materials and Methods: This is a descriptive, cross-sectional analytic study that pregnancy outcome in women presenting to Alzahra hospital for delivery was assessed. Variable studied included: enduced hypertension and diabetes in placental complication (placenta abruption, placental previa), preterm labor and premature rupture of membranes (PROM) was divided into 3 groups based on age 16 and 17-34 and 35.Pregnant women in two groups: 17-34 and 35 were matched according to parity. Data collection analysis was carried out with chi-square test by SPSS.9 software.
Results: In this survey, 6045 pregnant women with gestational age over 20 weeks were studied. Frequency of delivered women according to age group showed that 52 cases (%0.8) were 16 years, 5406(89.4%) were 17-34 years and other 587(9.8%)were 35.Data showed that there were significant differences between the three age groups and the risk of pregnancy-induced hypertension, gestational diabetes, placental abruption, placental previa, PROM, mal presentation, preterm labor and cesarean section (P<0/05).
In contrast, there wasn’t a statistically significant difference between advanced maternal age and the frequency of PROM (P>0/05).
Conclusion: It appears that pregnancy in age 35 comparing with teenage pregnancy has special risk factors. Awareness of the risks and complications has an important role planning for appropriate prenatal services.
