چکیده
مقدمه: لپتوسپیروز یک بیماری عفونی شایع مشترک انسان و حیوان است که درمناطق گرمسیری و نیمه گرمسیری شیوع بیشتر دارد. مخزن این باکتری در طبیعت جوندگان و بسیاری از حیوانات وحشی و اهلی هستند. اغلب این حیوانات پس از ابتلا به این بیماری تا آخر عمر خود حامل باقی میمانند و باکتری را از راه ادرار خود ترشح میکنند. باکتریهای ترشح شده میتوانند از راه خراشهای جلدی وارد بدن میزبان دیگر (حیوان و انسان) بشوند و چرخه بیماری راتداوم بخشند.
برای انجام یک مطالعه، همهگیری شناسی جامع در یک منطقه آندمیک، شناسایی لپتوسپیراهای شایع و بومی یک قدم اساسیست. به دلیل سخت رشد بودن لپتوسپیرا، جداسازی آن از نمونههای بالینی به روش کشت بسیار مشکل، وقتگیر و اغلب ناموفق است. به همین دلیل یک روش سرولوژیک به نام آزمون آگلوتیناسیون میکروسکوپی(MAT) برای تشخیص این بیماری و نیز شناسایی عامل آن در حد گونه و سروگروپ معتبر ترین روش محسوب می شود و در اغلب آزمایشگاههای مرجع لپتوسپیروز, متداول است.
هدف: شناسایی سروگروپهاوسرووارهای مولد لپتوسپیروز حاد انسانی در استان گیلان با روش MAT.
مواد و روشها : این مطالعه در سال 1382 با نمونهگیری از خون بیماران بستری در بیمارستانهای امام خمینی صومعهسرا، رازی رشت و 22 آبان لاهیجان که بر اساس علائم بالینی و طبق تشخیص پزشک معالج به بیماری لپتوسپیروز مشکوک بودند آغاز شد. سرم بیماران تا زمان آزمایش در فریزر 200- سانتی گراد نگهداری شده و در تابستان 1383 تمام نمونه سرمها ابتدا با روش الیزای نیمه کمی غربالگری شدند و سپس نمونههای مثبت با آزمون MAT مورد آزمایش قرار گرفتند.
نتایج: 282 نمونه سرم ابتدا توسط آزمون الیزای نیمه کمّی بررسی شدند وتعداد 130 مورد تراز IgM مساوی یا بالاتر از 1:160 داشتند که برای آزمونMAT در نظر گرفته شدند. 70 نمونه سرم تراز ≥ 160 در هر دو آزمون داشتند که همگی مثبت تلقی شدند و در مورد هر یک از آنها بالاترین تراز سرمی در آزمونMAT ملاک تعیین سرووار سبب ساز بیماری قرار گرفت.
نتیجهگیری : تفسیر نتایج MAT به دلیل واکنش متقاطع زیادی که بین سروگروپهای مختلف بویژه در نمونههای بالینی مربوط به مرحله حاد بیماری وجود دارد مشکل و پیچیده است. طبق تعریف کنونی CDC, یک تراز سرمی 200 ≥ اگر با علائم بالینی مطابقت داشته باشد تشخیص احتمالی را مطرح می کند. در این مطالعه با توجه به اینکه نمونه گیری فقط از بیماران دارای علائم بالینی مشکوک به لپتوسپیروز گرفته شده و پس از غربالگری بایک روش الیزای معتبر، فقط نمونههای مثبت با تراز IgM مساوی یا بالاتر از 1:160 مورد بررسی با آزمون MAT قرار گرفته بودند، با در نظر داشتن سه ملاک : مطابقت علائم بالینی، نتیجه الیزا و نتیجه MAT, ضریب اطمینان تشخیصی بالایی در نظر گرفته شده است و می توان سرووارها و سروگروبهای تعیین شده را در این منطقه شایع تلقی کرد.
Abstract
Introduction: Leptospirosis is a common Zoonosis, which is more prevalent in tropical and temperate regions. Rodents, wild and domestic animals are reservoir of Leptospirosis. Usually the infected animals are carriers for the rest of their lives and the bacteri is secreted from their urine. The secreted bacteria can enter a host (animal or human) and continue the circle of disease.
Objective: Diagnosis of leptospirsis according to clinical symptoms is difficult due to lack of pathogenic sign(s) and the similarities of its clinical features to some common febrile diseases, so laboratory is important in diagnosis. Leptospira is fastidious and its isolation from other clinical specimens is difficult, time consuming and usually unsuccessful, so MAT is the gold standard for diagnosis and serotyping of leptospira and is usual in all reference laboratories.
Materials and Methods: We performed this study in 1383 by taking blood sample from patients hospitalized in Emam Khomeini Hospital in Some- e-sara, Razi and 22 Aban Lahijan and had clinical symptoms and were suspected of leptospirosis. Sampling was done in spring and summer, which are prevalent seasons of leptospirosis in Gailan prorince. We stored all serums in- 200ºc until examination, and in summer of 1383 serum samples were screened by a Quantitative Elisa method to detect positive samples for doing MAT, and performed MAT to determine causative serogroups.
Results: 282 serum sample were all tested by quantitative Elisa and 130 cases had IgM titer equal or greater than 1:60 which were considered for MAT test. Seventy serum samples had titer ≤160 in both tests and all were positive and highest serum titer in MAT test was the determining criteria for causing disease.
Conclusion: Analyzing the results of MAT was hard and complex due to alternate reactions happening between different serogroups specially in clinical samples related to acute disease. According to CDC, a serum titer of ≥ 200 and if it correlates with clinical symptoms can present probable diagnosis. In this study, considering the samples were only taken from patients with clinical symptoms and suspected of leptospirosis and then after screening by Elisa, only positive samples with IgM titer equal or greater than 1:16 were assessed by MAT keeping in mind three criteria (Correlating clinical symptoms Elisa and MAT results) there is a high efficacy of diagnosis and determines servoirs and serogroups are prevalent in this area.
