جدا سازی هلیکوباکتر پیلوری به روش PCRاز آفت های دهانی بیماران مبتلا به آفت های راجعه دهانی در سال 1381 در شهر رشت

چکیده
مقدمه: آفتهای راجعه دهانی Recurrent Aphthous Stomatitis(RAS) اختلالی است که به صورت زخم هایی محدود به مخاط دهان مشخص می شود. بـه دلیل شباهت های بافت شناسی که بین زخم پپتیک (Peptic ulcer) و آفت های راجعه دهانی مشاهده شده و چون ضایعات آفت های دهانی اغلب به درمان با آنتی بیوتیکهای وسیع الطیف مثل تتراسایکلین پاسخ می دهند ، و نقش هلیکوباکترپیلوری که در زخم پپتیک مشخص گردیده، پیشنهاد شده که این باکتری ممکن است در توسعه زخم های آفتی راجعه دهان دخالت داشته باشد. با وجود این، فقط مدارک محدودی در مورد کلونیزه شدن یا نقش احتمالی هلیکوباکترپیلوری در زخم های آفتی دهان وجود دارد.
هدف: در این مطالعه خواسته شده است تا با انجام آزمایش PCR (Polymerase Chain Reaction) بر روی نمونه های برش زده زخم های آفتی به دنبال حضور احتمالی هلیکوباکترپیلوری در این زخم ها باشیم.
مواد و روش ها: دراین مطالعه از آفت های دهانی بیماران مبتلا به RAS ، به وسیله برسهای دندانی نمونه برداری شد، و به منظور تعیین هلیکوباکترپیلوری از نمونه ها، آزمایش PCR، که یک روش بسیار دقیق و حساس برای جدا سازی هلیکوباکترپیلوری در نمونه های زخم آفت های راجعه دهان و نمونه هایی از مکانهای دیگر دهان است، انجام گرفت. در کلیه این بیماران همچنین تست سرولوژی به روش ELISA برای تعیین میزان آنتی بادی IgG ضد هلیکوباکترپیلوری نیز به عمل آمد. بر روی تمام بیماران هر دو آزمایش PCR و ELISA انجام گرفت. بیماران مورد نظر، دارای آفت های راجعه دهانی فعالی بودند که از خرداد ماه سال 81 تا خرداد ماه سال82 به آزمایشگاه مراجعه می نمودند.
نتایج: 50 مورد ازبیماران مبتلا به RAS مورد مطالعه قرار گرفتند که محدوده سنی18 تا60 سال داشتند و میانگین سنی آنان 30/11± 38/32 سال بود، تعداد 26 نفر (52 درصد) از نظر تست ELISA مثبت شدند و از یک بیمار (2درصد) ، با انجام آزمایش PCR ، DNA هلیکوباکترپیلوری به دست آمد.
نتیجه گیری: طبق نتایج بدست آمده از این مطالعه، DNA هلیکوباکترپیلوری در زخم های آفتی این بیماران ،حتی در آنهایی که آنتی بادی(IgG) ضدهلیکو باکترپیلوری آنهادرمطالعه مثبت گزارش شده دیده نمیشود و این احتمال وجود داردکه این باکتری هیچ دخالتی در آفت های راجعه دهانی بیماران نداشته باشد.
Abstract
Introduction: Recurrent aphthous stomatitis (RAS) is a disorder, which is characterized by ulcers restricted to oral mucousa. Regarding the inspection of histological similarities between peptic ulcers and recurrent oral aphthous and the response of RAS to the broad-spectrum antibiotic treatment such as tetracycline and the identified role of H.pylori in peptic ulcers, the possibility of bacterial involvement in the progression of oral aphtous ulcers has been suggested. Hence, there are limited evidence available for colonization of H. pylori and its probable role in induction of oral aphthouse ulcers.
Objective: We aimed to distinguish the probable presence of H.pylori in brushed sample of oral aphthous ulcers by PCR method.
Materials and Methods: In this study, we sampled the oral aphtous ulcers with the toothbrush in patients with RAS. PCR which is a very precise and sensitive technique for isolation of H. pylori in ulcer samples of recurrent aphthous stomatitis and in samples from other parts of the oral cavity. Serologic tests such as ELISA were also done in all patients for determination of IgG antibody. Both PCR and ELISA were done in all patients. Evaluated patients were people presenting to the laboratory with the diagnosis of RAS, from the beginning of 2001 till the end of 2002.
Results: Study patients were between 18 to 60 years old and the average age among them was 32/38 11/30.
26 patients (52%) had positive ELISA test and we obtained H.pylori DNA in one patient (2%) in a total of 50 evaluated patients.
Conclusion: According to the results of this study, H. Pylori DNA can’t be found in aphthous ulcers of these patients, even in those with positive anti H. Pylori, anti body (IgG) results. And it is probable that these bacteria aren’t involved in oral recurrent aphthous ulcers.
