بررسی عوامل مرتبط با سردرد و کمر درد پس از بیحسی نخاعی در اعمال جراحی ارتوپدی

چکیده مقدمه: بیحسی اسپاینال به عنوان روشی متداول در جراحی شناخته شدهاست که از نظر عوارض جانبی بدنبال آن، در برخی موارد سر درد و کمر درد گزارش شدهاست. عوامل متعددی همانند شکل سوزن، اندازه آن، جنس و سن بیمار و تعداد موارد انجام شده در بروز آن دخیل هستند. هدف: بررسی عوامل مرتبط و اتخاذ روشهای تعدیل آن مواد و روشها: این پژوهش بهصورت مطالعهای توصیفی و آیندهنگر بر 200 بیمار 65- 15 ساله ASA کلاس 1 و2 کاندید جراحی ارتوپدی اندام تحتانی صورت پذیرفت. بیهوشی با سوزن 24 و لیدوکائین 5% و اپینفرین انجام شد. پرسشنامه بیماران از لحاظ سردرد-کمردرد قبلی یا سابقه بیهوشی نخاعی پر شد. همچنین، در مرحله پس از جراحی نیز از سردرد وکمر درد به فاصلهی یک روز، یکهفته و یکماه پس از جراحی پرسیده شد. سردرد یا کمر دردهای غیرقابل کنترل با درمانهای معمول با تجویز مسکنهای اوپیوئیدی درمان می شدند. آنالیز آماری توسط SPSS 16 صورت پذیرفت و P<0.05 معنیدار تلقی شد. نتایج: 25 نفر(5/12%) از بیماران سابقهی کمر درد و 13 بیمار(5/6%) نیز سابقهی سردرد معمولی داشتند. در آنالیز انجام شده میزان سر درد در یک روز، یک هفته و یک ماه پس از بیحسی به ترتیب (5/6%، 5/3% و 1%) و هم چنین میزان کمردرد بیماران به ترتیب 16%، 9% و 5/3% بدست آمد که البته دو معیار فوق تفاوت آماری معنیداری نداشتند در بیماران دارای سابقه بیحسی اسپانیال و مشکل کمردرد تفاوت آماری معنیداری بدست آمد(P<0.001 ). چنین یافتهای در خصوص سردرد نیز ملاحظه شد. نتیجهگیری: به نظر می رسد بین سردرد و کمر درد بیماران با سابقه بیحسی اسپانیال ارتباط معنیداری وجود داشتهاست. کمر درد قبلی و جنس تأثیر بارزی بر کمردرد پس از عمل داشت که در مورد سردرد چنین یافتهای بدست نیامد.
Abstract Introduction: Spinal Anesthesia (S.A) is currently becoming a more common and popular method for orthopedic surgeries. Unfortunately, Backache (BA) and Post Dural Puncture Headache (PDPH) are both frequent and troublesome. Several factors (including needle shape and size, frequency rates of puncture attempts, and age and gender) have been shown to affect the rates of such complications. Objective: To determine these factors and evaluate some modalities to prevent them. Materials and Methods: This is a prospective study on 200 patients-15 to 65 years old with class type 1 and 2 A.S.A(American Society of Anesthesiology), who were candidates for orthopedic lower extremity surgeries. Anesthetic procedure was performed using a standard 24G needle Lidocaine 5% plus Epinephrine. Symptom questionnaire was filled for the history of Headache and Backache and history of former S.A. post-op puncture complications of 1st day, 1st week and 1st month were added to the forms. Statistical analysis was performed using SPSS version 16 and the results were considered significant at p<0.05. Results: Rates of BA and PDPH were 12.5% and 17%, respectively. Post anesthetic BA for first day, week, and month were 16%, 9%, and 3.5%, respectively and PDPH were 6.5%. 3.5% and1% respectively with no significant differences. History of former spinal anesthesia with BA and PDPH revealed notable differences (p<0.001). Conclusion: The results of this study support the idea that history of previous spinal anesthesia could be a triggering factor for backache and headache in prior Spinal anesthetic procedures.
