Background Nurses are the main part of the health care system. Hospital managers, with appropriate leadership styles, can provide the environment to increase nurses’ work engagement and motivation. 
Objective This study aims to investigate the effect of perceived transformational leadership on nurses’ work engagement, considering the mediating role of organizational identification.
Methods The methods of this research is descriptive and correlational study. Data were analyzed using structural equation modeling with SPSS software, version 26 and SmartPLS software, version 3.
Results The perceived transformational leadership had a significant effect on organizational identification (t=9.288, effect size=0.473) and job engagement (t=3.459, effect size=0.203) of nurses. The organizational identification had a significant effect on work engagement (t=8.490, effect size=0.482). The results of the Sobel test showed that the mediation effect of organizational identification was 0.529.
Conclusion The transformational leadership perceived by nurses affects their organizational identification and work engagement. Therefore, by improving the transformational leadership capabilities of hospital managers, the organizational identification and work engagement of nurses can be improved.
